POLICIES ON LOST MATERIAL/S

A loaned material (purchased or donation) that is lost must be reported immediately to any of the Library Staff. It may be:

  1. Replaced with the exact title, author, edition or year, plus P100.00 processing fee and accumulated fine/s.
  2. Replaced with similar title having the latest edition, plus P100.00 processing fee and accumulated fine/s.
  3. Paid according to the price of the book, plus P100.00 processing fee and accumulated fine/s.
  4. Paid with 150.00 (for donations), plus P100.00 processing fee and accumulated fine/s.

 A lost material must be settled within one week after the case was reported to avoid accumulation of fines.

 For paying and replacing lost material/s, follow the procedure below:

  1. Report loss to the Section Head.
  2. A form shall be issued by the Section Head bearing the assessed fees to be paid.
  3. Present the form to the Accounting Office and pay the stated fees.
  4. Bring the receipt to the Section Head.
